When you use a word such as "retard" in a context as such, it tells more about yourself than the guy you are trying to insult.
I never use a sickness as an insult. In fact, I think it is much more effective not to swear at all, which shows the superiority over the other one. If someone yells at me, cusses, etc. I only look into his face and smile.
